Based on review of Patient's Rights information, admission materials and interview, it was determined the Facility failed to inform the patient of the right to lodge a complaint with the State Agency and provide the State Agency contact information. The failed practice by the Facility to properly inform patients of the complaint process and State Agency contact information potentially affects any patient admitted to the Facility. Findings follow:
A. Review of Patient's Rights materials and documents provided to the patient upon admission revealed no evidence of the patient being inform of the right to lodge a complaint with the State Agency or the State Agency address and contact information.
B. On 02/19/14 at 1120, findings were confirmed by the Director of Quality and Risk Management.
