Based on document review and interview, the facility failed to meet the frequency of therapy visits in the treatment plan in 1 of 10 (patient 9) medical records reviewed.

Findings include:

1. Review of patient 9's medical record lacked documentation of three physical therapy visits per the initial evaluation and treatment plan on 11/24/2023, which indicated four visits per week.
a. Patient was seen for three visits between 12/1/2023 to 12/7/2023.
b. Patient was seen for two visits between 12/8/2023 to 12/15/2023.

2. Interview with A7 (Director of Acute Care) on 12/28/2023 at approximately 3:30 pm confirmed Patient 9's medical record lacked documentation of three required physical therapy visits to meet treatment plan and frequency and documentation for reason visit was missed.
