Based on record review and interview, the hospital failed to protect the rights of each patient. The deficient practice is evidenced by failure to ensure two staff members were present during a nursing admission assessment, which included a body search, for 2 (Patient #1, #3) of 3 sampled patients.
Findings:
Review of the form titled "Nursing Admission Assessment" revealed it contained an area for patient body search. Further review of the form revealed a space for two signatures, indicating that two staff members were present for the physical exam. Review of the forms for Patient #1 and Patient #3 revealed there was only one signature on the form for each patient.
During an interview on 04/29/2025 at 2:15 PM, S1Director of Performance confirmed that Patient #1 and Patient #3's "Nursing Admission Assessment" forms did not have two signatures on them. S1Director of Performance also revealed that two staff of the same sex as the patient are to be present for the assessment, which includes a body search, and both staff are to sign off on the assessment forms.

Based on record review and interview, the nursing staff failed to ensure a personal property record form was completed upon discharge for 1 (Patient #1) of 3 sampled patients.
Findings:
Review of the record for Patient #1 revealed an admit date of 02/12/2025. Review of the personal property record form dated 02/13/2025 revealed the patient admitted with several personal items, including three blouses/shirt, three pants/shorts, one pair of shoes, one sweater, four pairs of underwear, three bras, three socks, one deodorant, one toothbrush, one lotion, and four books .
Further review of the personal property record form revealed there was no documented evidence at discharge that the patient's personal items were returned. There was a signature spot on the form, indicating that the items were returned to the patient at discharge, but it was blank.
During an interview on 04/29/2025 at 2:20 PM, S1Director of Performance confirmed Patient #1's personal property record was not signed at discharge by the patient/representative or a staff member. S1Director of Performance further confirmed that there was no documented evidence that the personal items that the patient brought upon admission were returned to her upon discharge.
