Based on observation and interview, the facility failed to maintain vertical openings through the ceiling/floor assembly in numerous locations, on one of two floors of the facility.
Findings include:
1. Observation on December 2, 2010, at 10:20 AM revealed that the Basement Electrical Room had numerous unsealed penetrations through the floor slab above, inside and around conduits and pipes.
Interview with the Maintenance Director on December 2, 2010, at 10:20 AM confirmed the unsealed floor penetrations.

Based on observation and interview, the facility failed to maintain the proper fire resistance rating of smoke barrier walls in two locations, on one of one floor within the facility.
Findings include:
1. Observation on December 2, 2010, between 9:50 AM and 10:42 AM, revealed the following unsealed smoke barrier penetrations:
a) 9:50 AM, an approximate two-inch penetration above the double corridor doors, located above the exit sign at door jam 327;
b) 10:42 AM, around silver MC cables, above the double corridor doors at door jam 287.
Interview with the Maintenance Director on December 2, 2010, at 10:42 AM confirmed the unsealed penetrations.

Based on observation and interview, the facility failed to ensure that portable fire extinguishers, provided for the protection of cooking appliances using vegetable or animal oils and fats, were maintained in one location, on one of one floors of the facility.
Findings include:
1. Observation on December 2, 2010, at10:40 AM revealed that the "K" type fire extinguisher, used in the Kitchen, lacked signage identifying the use of the extinguisher as a secondary backup means to the automatic fire suppression system.
Interview with the Maintenance Director on December 2, 2010, at 10:40 AM confirmed the missing fire extinguisher sign.

Based on documentation review and interview, the facility failed to ensure that monthly inspections were performed on the building kitchen suppression system, located on one of one floors.
Findings include:
1. Review of documentation on December 2, 2010, between 8:15 AM and 9:15 AM, revealed that facility lacked documentation that monthly "quick checks" were performed on the kitchen suppression system.
Interview with the Maintenance Director on December 2, 2010, at 9:15 AM confirmed the missing inspections.

Based on observation and interview, the facility failed to maintain electrical wiring systems in two locations, on one of two floors of the facility.
Findings include:
1. Observation on December 2, 2010, at 9:50 AM revealed that an exit sign, located above the double corridor doors at the Ambulance Entrance, was wired with nonmetallic wiring.
Interview with the Maintenance Director on December 2, 2010, at 9:50 AM confirmed the improper wiring.
2. Observation on December 2, 2010, at 10:00 AM revealed that a microwave and coffee pot were plugged into a surge protector, in PRISM Room 338.
Interview with the Maintenance Director on December 2, 2010, at 10:00 AM confirmed the improper use of the surge protector.
