Based on review of 20 patient records, there were 18 of 20 patient records that contained physician verbal or telephone orders that the facility failed to ensure had been signed within the required 48 hours.

Findings were:

A review of 20 patient records revealed that 18 of 20 failed to have physician orders authenticated within 24 hours. Records 2 through 10 and 12 through 20 all had unsigned physician orders. The record with the most unsigned orders was #19 with 48; record #12 had 19 unsigned orders; and record #16 had 15 unsigned orders.
The above findings were confirmed in interview with the director on 7/10/12.
