Based on observation, interview and record review, the facility's governing body failed to comply with its own education requirement policy applicable for contracted staff member in 1 of 3 sampled staff members (K).

On 12/22/2014 at 09:05 a.m. staff Registered Nurse was observed caring for Room # 201.
During an interview on 12/22/2014 at 09:15 a.m. with Contract Nurse (K), the Surveyor asked the Contract Registered Nurse for her length of service, and describe the patient assignment for the day. Contract registered Nurse (K) described she was coming back and forth to Kindred Hospital at this facility to provide care to patients through an agency she is working for.
Review on 12/22/2014 of the employee (K) ' s file, it stated that the employee was hired on 04/24/2005 by the Kindred Hospital, and had a Nursing Skills Checklist on 03/15/2013. Another employee file was sent by the Facility to the Surveyor on 12/23/2014, it stated that the employee ' s re-orientation date was on 12/22/2014---the date of the unannounced survey.
An interview made on 12/22/2014 at 09:50 a.m. with the Director of Nursing (N), requested for an updated Nursing Skills checklist file, and said " I have contacted the Staffing Agency and they are still on the process of looking for her updated annual Nursing skills checklist. "
Review of the facility's contracted Policy and Procedure # H-ML 02-2013 related to HR/Licensure/Education for Non-Employee Contracted Personnel adopted by the facility directed the facility as follows on page 3 item 2.d. " Non-employee/contracted personnel who work consistently for one year will be provided with and required to complete all required annual training the same as hospital employees. " Page 4 item #4 " Orientation to the Hospital for ALL non-employee/contracted personnel and volunteers hsall include but not be limited to the following a. Information received in General Hospital Orientation ... "
