Based on observation and staff interview it was determined the facility failed to provide properly installed exit directional signs.

The findings were:

During a tour of the facility with Staff M on 06/10/13 from 9:50 a.m.to 12:00 p.m. It was observed the facility had exits directional signs that are improperly installed or not need at the cross halls on the first and second floors. Signs number 039 need to be removed from the First floor cross hall (not needed). Sign numbers 013 and 015 need to be removed from second floor cross halls (not needed). Sign numbers 014 and 016 are not correctly oriented to direction of exits.

This finding was confirmed by Staff M at the time of discovery.
