Based on observation during the facility walk through on 04/24/2012, accompanied by the hospital administrator and director of maintenance, that when the fire alarm system was tested, the cross-corridor smoke barrier doos at the nursing suite failed to release from their magnetic hold-open devices. the doors remained open during all testing of the fire alarm system.

Based on observation during the facility walk through on 04/24/2012, accompanied by the hospital administrator and director of maintenance, it was noted that there were no manometers or manahelic gauges mounted at the HVAC final filter (95%) filter banks to monitor static pressure differential, as required.

Based on observation and discussions with the hospital administrator and director of maintenance, during the facility walk through on 04/24/2012, it was noted that there was not a remote annunciator panel for the emergency generator, located at the engineering office, as required. there was an annunciator panel at the 24 hour nurse station in the nursing unit.

Based on observation during the facility walk through with the hospital administrator and director of maintenance, on 04/24/2012, it was noted that the exterior sides of all emergency system electrical panels (Critical, Life Safety, and Equipment Branches) lacked color coded (red) identification plates, as required. the panel labels were labeled in black, the same as all "normal" powered electrical panels.
