During the tour of the facility on June 3, 2020 between 9.00 am and 10.30 am, surveyors observed two (2) ceiling tiles missing over the top of a chiller unit in the facility's kitchen area. These missing tiles could allow smoke and flames to travel throughout the kitchen and kitchen storage area. The missing tiles could also delay activation and notification of the fire alarm and sprinkler systems. The facility's staff accompanying the surveyors were immediately notified that these missing tiles needed to be replaced and all ceiling tile assemblies should be checked for missing or misaligned tiles. Surveyor verified the corrective actions during an additional tour of the kitchen area on June 9, 2020.

During the tour of the facility on June 3, 2020 between 9.00 am and 10.30 am, the exposed fire sprinkler head, in front of the food warmer in the facility's kitchen area, was found to be extremely dirty. The facility's staff accompanying the surveyors were immediately notified that this sprinkler head needed to be cleaned or replaced to ensure proper operation and coverage of the affected area. Surveyor verified the corrective action during an additional tour of the kitchen area on June 9, 2020.

During the tour of the facility on June 3, 2020 between 9.00 am and 10.30 am, the surveyors observed that the Class K fire extinguisher in the facility's kitchen area, near the office, was not identified with a sign. The facility's staff accompanying the surveyors were immediately notified that the sign on this extinguisher needed to be replaced. Surveyor verified the corrective action during an additional tour of the kitchen area on June 9, 2020.
