Based on observation the facility failed to provide acceptable smoker barrier separations.
The inspector observed while accompanied by the Director of Plant Operations during the hours of the inspection from 11:30 am to 3:00 pm that there were penetrations in the smoke barrier at the following locations: 1) above the vending machine adjacent to a nurse station, 2) in the staff lounge, 3) in the HBO sterilization room, and 4) in information management.

Based on observation the facility failed to provide an acceptable cross referencing of the fire alarm control panel and the panel and breaker supplying power.
The inspector observed while accompanied by the Director of Plant Operations during the hours of the inspection from 11:30 am to 3:00 pm that the breaker that supplied power was not colored red.
