Based on visual observation the facility failed to provide doors with self-closing devices in an exit passageway that automatically closes throughout the smoke compartment or entire facility.
Findings:
During the facility tour between the hours of 9:00 am and 10:30 am it was observed that
1/ 5th floor double doors located in hall leading to dining area are not closing and latching in their frames
2/5th floor double doors located in elevator lobby and leading to dining area are not closing and latchin in their frames.
Interview with Hospital administration revealed that the facility was not aware that the doors were not closing and latching providing automatically closing doors at all times in accordance with 19.2.2.2.7.

Based on visual observation the facility failed to provide hazardous areas with doors that are self-closing or automatic-closing in order to resist passage of smoke.
Findings:
During the facility tour, between the hours of 9:00 am and 10:30 am it was observed that
1/6th floor oxygen door is not automatically closing and latching in its frame,
1/ 6th floor biohazard door is not automatically closing and latching in its frame
Interview with Hospital administration it was revealed that the facility was not aware that the oxygen storage door and biohazard storage doors were not closing and latching automatically in order to provide smoke resistive storage rooms.
