Based on observation and staff interview, the facility is not providing an unobstructed all-weather surface from each exit to a public way (an area of safety). This would affect all patients and staff in the facility's 100 patient corridor. This facility has a capacity of 21 with a census of 7 patients.
Findings are:
1. On 1/12/15 at approximately 1315 hours, the path of egress from the east exit door of the 100 patient corridor was obstructed with snow and ice that the facility failed to remove from in front of the exit door on the all-weather surface. According to the facility layout, this was a required emergency exit and the facility ' s fire plan stated that occupants, including those using walkers and wheelchairs, would use the exits in the event of an emergency that required evacuation. An interview of Maintenance Staff A had verified the obstruction and advised that he was unaware that snow and ice had blown in and was obstructing the exit and exit access.
