The findings were:
During a tour of the facility with staff M on 01/24/2017 between 09:30am and 11:45am observation revealed 6 conduit penetrations in the elevator machine room not sealed with an approved fire stop material.
These findings were confirmed by staff M at the time of discovery.

Based on observation and staff interview it was determined the facility failed to provide smoke barriers with at least a 30 minute fire resistance rating and would resist the passage of smoke. This could place all 47 residents at risk in the event of a fire in the facility
During a tour of the facility with staff M on 01/24/2017 between 09:30am and 11:45am observation revealed unsealed penetrations, penetrations sealed with sheet rock mud, and patches improperly constructed above 3 of the 5 fire and smoke barrier doors in unit 2 located on the north end.
These findings confirmed by staff M at the time of discovery.
