Based on observation and interview, the facility failed to maintain 1 of 11 egress exits (large conference room) to be used in the event of an emergency evacuation.

Findings:

1. A tour of the facility was conducted on 10/01/25 between 9:25 a.m. and 2:30 p.m., and the following observation was made:

The sidewalk outside the large conference room did not continue to the parking lot or public way.

2. The maintenance director was present during the tour of the facility and he confirmed the sidewalk did not continue to the parking lot or public way.
