|Based on document reviews and staff interviews it was determined the hospital failed to follow their own policy for Patient Complaints. The failure has the potential to adversely affect the rights of all patients.
1. Review of the hospital policy titled " Patient Complaints, effective date 9/14/16, revealed in part: At the time of registration, documents will be made available for each patient regarding their right to file a complaint/grievance and the hospital's mechanism for responding. The brochure will also include the address and phone number of the West Virginia Office of Health Facility Licensure and Certification".
2. A review of three (3) hospital Admission packets on 10/23/17 at 11:30 a.m. revealed no credible evidence this information was given to patients.
3. An interview was conducted on 10/23/17 at 11:40 a.m. with the Risk Manager Registered Nurse. She agreed the Admission packets did not contain the information titled " Patient Complaints". This was cited at A 0118.
