|Based on document review and interview the hospital failed follow policy/procedure regarding oral complaints.
Policy "0900, Handling, Resolution, and Dissemination of Information from Patient Grievances" allows patients to provide oral grievances/complaints.
Document titled, "Brookhaven Hospital Grievance Filing Notice" was reviewed. The document requires all complaints and/or grievances to be written on the "GRIEVANCE AND RESOLUTION REPORT" form.
Interview with Administrator and DON confirmed their process contradicts their policy, that states complaints can be orally communicated.

|Based on document review and interview the hospital failed to provide ongoing assessments of patients, as required by physician orders, to provide appropriate delivery of care.
Review of patient medical records showed 18 of 23 patients did not have ongoing assessment of patients as ordered.
Orders were written for vital signs (blood pressure, pulse, temperature, and respiration rate) to be taken BID (twice daily). Assessment of vital signs were recorded at intake only.
After intake only blood pressure and pulse were shown in the records.
Interview with DON confirmed that patient assessments were not being conducted and recorded as ordered by the physican.
